At a real site, an incident review turned up something nobody wanted to find: a critical control had been bypassed for months. Not sabotaged, not missed in an audit, just quietly not working, while the paperwork behind it stayed current the entire time. The bowtie diagram for that risk was airtight. On paper, the promise was being kept. On the ground, it had stopped being true, and no one had noticed.
